7/18/2023 EnchantaVaughn Approved Upon arrival, client was observed walking with peers and counselor. Client was praised for walking and not running. Client went to his age group and started morning activity. Client was observed initiating conversation. Client was observed displaying good social skills in the camp environment. Client waited patiently for his turn when participating in the game room. Client was observed following camp rules and being respectful. Client was educated that it is important to interact with others and learn to be sociable to assist him when he transtions back to the school setting in the upcoming months.(EOD) As client went into the final part of the day, he was observed walking quietly to the next activity. Client presents frustration when faced with conflict and was given a pep talk to ignore the distractions and try not to follow behind others who are doing wrong. Client was acceptive and implementing conflict resolution skills. Client was seen in the teen room engaging in team building activities, such as playing video games and working as a group. Client was seen contributing to group discussion and sharing his ideas. Client was in a good mood, as evidenced by his facial expression and body language. Client prepared for dismissal by cleaning up and putting away supplies. Client was polite and respectful to authority, listening and following directions. No redirection was needed at this time. QMHP observed client while in the classroom in efforts to determine the need for intervention. QMHP provided a positive hand gesture such as a thumbs up in efforts to show approval of clients current actions. QMHP did not have to redirect client. QMHP did not have to intervene with clients behaviors as he demonstrated adherance to classroom rules and expectations. QMHP provided a simple hand gesture to encourage client to continue with the positve classroom behaviors. ((EOD) Client was able to reduce negative behaviors and focus on the camp activity. QMHP will continue to assist client with managing anger and frustration by reviewing and modeling effective coping skills. QMHP encouraged the client to respect his instructor. QMHP reviewed school rules and expectations with client before walking him into his classroom, ensuring that he was safe and comfortable. QMHP reviewed expectations while transitioning with client. QMHP prompted the client to be mindful of his behaviors and how they affect others. QMHP supported client involvement in selected activities. QMHP processed with client about what he learned from activity after the review from the instructor. QMHP praised client for his cooperation inspite of not wanting to do the activity. QMHP praised client, gesturing for him to remain on task. QMHP cheered on client as he decided to challenge himself with new ideas. QMHP supported client participation and recognizing his own behaviors and correcting himself without guidance. Client expressed positive self talk, modeled positive behavior, and good communication skills during role play activity. Client learned to avoid negative and defiant behavior and comply to rules. Client was seen following directions. Client joined session to utilize and learn new interventions and strategies. Client agreed to stay focus and have a good day. Client displayed a lot of low self esteem with interacting with other campers. Client demonstrated how to verbalize his feelings without yelling and being aggressive. Client was able to speak calmly and demonstrate his improved verbalization skills while conversing with other mental health professional. Client did not display any outburtsts during the role play exercise. Client appeared to be learning from his mistakes and correcting them throughtout the day. Client responded well to praise and encouragement. (EOD) Client saw that he was the only one out of line and went back to his spot. Client worked well in groups and was seen sharing his supplies with his peers during a coloring activity. Client displayed appropriate communication skills in the arcade rooms by using positive social skills toward peers and adminstrative. Client seem to be enjoying camp and getting to know others. Client was polite and courteous to staff. Client was happy to be on track and displayed cheerful emotions. Client said he was aware of his expectations and camp rules. Client expressed a good temper when given prompts to follow. Client responded well to praise and support. Client presents appropriate interaction in camp while participating in all activities.
